Staff-child ratio Classroom 1: (3-year-olds); 9 children present with 1 teacher<br/> Classroom 2: (3-year-olds); 6 children present with 1 teacher<br/> <br/> Classroom 3: (4-year-olds); 17 children present with 1 teacher (not in staff-ratio, classroom should have 1 teacher and 1 aide)<br/> Classroom 4: (4-year-olds); 15 children present with 1 aide (no qualified teacher)<br/>

Staff-Child Ratio On February 6, 2024 the worker conducted an unannounced visit to Alphabetland-Newtown and observed the (4 year old) classroom with 20 children present with 2 aides.<br/> No teacher qualified staff in the ratio.<br/>

Name, address, and phone numbers of persons authorized to take child from facility At the time of the visit the above-mentioned items were kept in an emergency binder, however the worker was unable to locate 5 children's emergency forms on site.<br/> <br/> On February 14, 2024, the worker found the missing emergency contact forms at the Waipahu office.<br/>
Inspection of facility, activities, records, staff On February 13, 2024, the worker made an unannounced visit to Alphabetland Preschool Newtown. The campus has a playground, office, kitchen, and 4 classrooms. 2 classrooms are not in use and are utilized as storage.<br/> The worker inspected the classrooms, kitchen and playground.<br/> <br/> The worker also reviewed the children's records and found 5 children's emergency contact forms missing from the emergency contact binder located at the Newtown facility.<br/>

Health information concerning the child The worker found the immunization information on child#1 missing along with the DHS 908.<br/> <br/> On February 28, 2024, the DHS 908 was submitted to the worker along with the Form 14, which was missing immunization shots. Administrative staff verified that the child has not been to school since February 13, 2024 and could possibly be terminating enrollment.<br/>
Health record on each child On February 14, 2024, the worker went the Waipahu office to review the children's health records and found child#1 missing immunization records.<br/> <br/> On February 28, 2024, the DHS 908 was submitted to the worker along with the Form 14, which was missing immunization shots. Administrative staff verified that the child has not been to school since February 13, 2024 and could possibly be terminating enrollment.<br/>
Activities and premises minimizes risk of accidental injury The worker observed the one of the (3 year old) classroom had two entrances blocked, internally within the classroom and externally outside of the classroom that prevented fire escape access.<br/> The internal door was blocked with office supplies and the exterior exit was blocked with cement blocks converted into a sitting area.<br/>
Written permission from parent/guardian for child to receive emergency care At the time of the visit the above-mentioned items were kept in an emergency binder, however the worker was unable to locate 5 children's emergency forms on site.<br/> <br/> On February 14, 2024, the worker found the missing emergency contact forms at the Waipahu office.<br/>
Physical exam clearance 3- staff are missing the DHS 984 medical report forms.<br/>
TB clearance 3- staff are missing TB clearances.<br/>
Sanitation inspection The facility has a current food establishment permit form the Department of Health that expires on April 14, 2024.<br/> Food establishment permit number#000577<br/> Risk Category:3<br/> <br/> The worker observed that the facility is licensed as a risk category 3, however an aide was observed preparing lunch. The worker confirmed with the director that this facility cooks and prepares food on site daily.<br/> The worker advised the director that they may want to submit a new application for risk category 2.<br/> The February 2024 menu for the day was: Beef Stew, Brown Rice, Fresh Potato & Carrots and Peaches.<br/>
