Staff member observed not using positive behavior supports. A licensee shall ensure that all staff use prevention strategies, appropriate redirection rather than restraint, and positive developmentally-appropriate methods of behavior supports for children, which encourage self-control, self direction, positive self-esteem, social responsibility, and cooperation. Staff member was terminated, and OCCL was sent the termination letter.

The Office of Child Care Licensing received a complaint on February 26, 2025, and additional information from the same Reporter on March 5, 2025, regarding allegations of failing to provide the required physical activity and outdoor play opportunities for children, incorrect staff-to-child ratios, not providing water to children as needed, and not following diapering procedures. An investigation was conducted on March 5, 2025. Based on the interviews, this complaint is substantiated for failing to provide the required physical activity and outdoor play opportunities for children. Two staff stated they have not gone outside since the end of last summer. Based on interviews and a review of attendance records, there is no evidence to substantiate this complaint for incorrect staff-to-child ratios, failing to provide water, or failing to follow diapering procedures. All documents reviewed showed classrooms maintained correct staff-to-child ratios. All staff interviewed stated water is available throughout the day in all classrooms. Classrooms were observed to have water available at the time of the visit. Documentation reviewed and staff interviews showed checking and changing of diapers is conducted as required.

On August 17, 2022, OCCL received a complaint regarding allegations of not feeding an infant appropriately. An investigation was conducted on August 19, 2022. Based on the results of this investigation, this complaint is substantiated because infant daily logs from August 10, 2022, and August 11, 2022, show Child #1 was not fed according to his monthly infant feeding schedule. The child was not fed the proper amount because he was transitioning from using a bottle to a cup and often refused the cup.

The incident was self-reported by the administrator. Staff member was seen on camera spraying children with a spray bottle. Staff member was terminated.
Ensure indoor space is clean. Vents in giraffes need to be cleaned.

Ensure staff files are complete. Staff 31 needs annual review of safe sleep and abuse and neglect, staff #2 needs annual review of safe sleep, staff #3 needs health appraisal and TB, staff #4 needs CPR/FA and health appraisal with TB, staff #5 needs CPR/FA and health appraisal with TB.
Ensure supervision and direct observation of children is provided at all times. Staff #1 and Staff #2 did not do this when they left a child alone in a classroom. This was corrected when the center administrator reported the two staff members were suspended.
Ensure floors, walls, and surfaces are cleanable and in good repair. Repair peeling paint on the wall in the Elephants room.
Ensure all staff use positive developmentally-appropriate methods of behavioral support for children. LS Flowers observed video footage of Staff #1 picking up a child by one arm and pushing another child across the surface of a table. This was corrected when Staff #1's employment was immediately terminated.
Ensure proper staff-to-child ratios are maintained at all times. At the time of the visit, the staff member in the infant classroom exited the classroom to enter the other infant classroom.
Ensure staff attendance is documented. At the time of the visit, one staff member was not signed into the Bunnies/Infants classroom.
Ensure child files contain all required elements. A blood lead screening is needed for two children.
Ensure staff and children wash their hands after diaper or toileting. At the time of the visit, a staff member in the Bears classroom did not wash a child's hands after diapering.
Submit the following to complete staff files: medication administration and a DEEDs certificate for one staff member.
