On March 4, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of improper security procedures, the center not being clean, as well as evidence of mouse droppings throughout the center. A visit was conducted on March 6, 2024, to investigate this complaint. This complaint is substantiated for not having proper security procedures in place. Upon arrival, the door to the church office was open. LS Brady was able to access the child care center because the church office door was unlocked, and the interior church door was also propped open. Both the church office door, and the interior church door that connects to the child care center should always remain locked and closed. There is no evidence to substantiate this complaint for the center not being clean. LS Brady conducted a walkthrough to ensure there was no evidence of mouse droppings. LS Brady also collected and received a copy of the exterminator s contract for preventative treatments and received a copy of the housekeeping daily schedule the staff follows throughout the day. The floors in the center did not appear to be dirty. All staff interviewed stated they are responsible for cleaning their own classrooms. The center appeared clean, and all staff denied ever having any issues with mice in the center.

Ensure hazardous materials are started inaccessible to children. Remove cleaners from the bathroom shelf. Lock classroom door in two-year classroom closest from staff belongings and cleaners.
Ensure cooling equipment shielded. Remove the fan in two-year-old classroom.
Ensure sufficient quantity of equipment in each classroom. -Ensure children have available toys for use in the two-year-old classroom. -Replace or repair the chair in the three-year-old classroom.
Ensure walls are cleanable. Clean/wipe down walls in the three year old classroom and one year old classroom. Ensure cabinet doors are cleanable and repaired.
Ensure refrigerator is 41 degrees on below.
Ensure fall zones meet requirements. Rake mulch to ensure fall zones are corrected.
Ensure there is emergency water for all children enrolled.
Ensure staff files are completed. Staff #1 needs to be fingerprinted since they turned 18. Staff #2 needs OCCL health and safety training.
