The Office of Child Care Licensing received a complaint on October 06, 2022, regarding staff yelling and mistreating children. An investigation was conducted on October 11, 2022. There is no evidence to substantiate this complaint based on interviews conducted and interactions observed between staff and children and families during the investigation.

On August 12, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding health and safety concerns, including unsanitary diaper-changing stations, maggots in trash cans, broken playground equipment, and failure to follow exclusionary practices outlined in DELACARE regulations. The complaint also alleged improper staff-to-child ratios as well as inappropriate staff interactions with children during water play. An investigation was conducted on August 18, 2025. There is no evidence to substantiate the complaint for the allegation of improper staff-to-child ratios. Administrator #1 provided Licensing Specialist (LS) #1 with attendance records for the week of August 11 15, 2025, which showed all classrooms were maintaining proper ratios in accordance with DELACARE regulations. There is no evidence to substantiate the complaint for the allegation of unsanitary conditions. LS #1 observed the program s three diaper-changing stations, all of which were properly maintained with required supplies. LS #1 and Licensing Supervisor #1 inspected all trash cans, and no evidence of maggots was observed. There is no evidence to substantiate the complaint for the allegation of improper discipline or inappropriate engagement during water play. Administrator #1 and all three staff members interviewed denied witnessing staff force children into water play or cause them distress. Staff members consistently reported that children who chose not to participate were either removed from the activity or supported in engaging with alternative options. Staff also denied observing any staff member photographing children who were crying or ridiculing them. There is no evidence to substantiate the complaint for the allegation of broken playground equipment creating safety hazards. LS #1 and Licensing Supervisor #1 inspected the outdoor play areas. While some plastic equipment showed signs of wear, no items posed a safety risk to children. There is no evidence to substantiate the complaint for the allegation of failing to follow child health exclusion practices or notify parents regarding the presence of hand, foot, and mouth disease. Administrator #1 provided LS #1 with communications documenting parent notification for a child later diagnosed with hand, foot, and mouth disease. Administrator #1 explained the child, who had severe eczema, had recently returned from the beach, and symptoms were not visible until bumps appeared later in the day. Once symptoms were presented, the child was immediately excluded, and the family was notified via the Bright Wheel application. Administrator #1 provided screenshots verifying this communication.

Disposition: No Evidence to Substantiate
On October 25, 2024, a complaint was called into the Office of Child Care Licensing (OCCL) regarding not providing communication about an injury, not answering the center telephone, not having an administrator for a long period of time, and having a child use a mat for napping before 12 months of age without permission. This complaint is substantiated for not providing communication regarding an injury because although an injury report was written, Parent #1 was not verbally notified or given the report upon pick-up. There is no evidence to substantiate the allegation of not answering the center's telephone because LS #1 observed Owner #1 answering the phone several times throughout the visit. Owner #1 noted Parent #1 and Parent #2 also called multiple times after hours when no one is available to answer the phone, and would reach out via the Brightwheel application, where they would receive a prompt response. There is no evidence to substantiate the allegation of Child #1 using a mat/cot for napping before twelve months of age. Documentation was provided to LS #1 supporting the transition of Child #1 after their first birthday as well as mat/cot permission signed by Parent #2. While the program s administrator resigned in August 2024, Owner #1 is qualified as an Early Childhood Teacher, handles administrative duties regardless, and has been present every day, open to close to assist the staff and run day-to-day operations. Owner #1 notified OCCL as required of the resignation of the administrator and has been in communication with LS #1 regarding the challenge of obtaining a newly named administrator, including showing documented efforts to do so. There is no evidence to substantiate children not having updated health appraisal or immunization requirements; all children in Child #1 s classroom were observed to have up-to-date health appraisal requirements.

The Office of Child Care Licensing received a complaint on September 28, 2022, regarding classrooms being out of ratio, unqualified staff being alone, inappropriate interactions between staff and children, and mice in the building. An investigation was conducted on October 11, 2022. There is no evidence to substantiate this complaint for classrooms being out of ratio because all classrooms were in ratio and attendance records reflected classrooms being in ratio. There is no evidence to substantiate this complaint for unqualified staff being alone because all classrooms had qualified staff present and attendance records reflected qualified staff being in the classrooms. There is no evidence to substantiate this complaint for inappropriate interactions between staff and children because all staff interactions observed with children were appropriate. There is no evidence to substantiate the presence of mice in the building because no evidence of mice or their droppings was observed.

A complaint was received on August 24, 2022, regarding the allegation there was an infestation of mice in the center as well as mice eating food being served to children. An investigation was conducted August 26, 2022. The complaint was substantiated because LS Brady observed a small amount of mouse droppings in between the cabinets on the floor of the kitchen. There was no evidence of mice eating food being served to children. A concern noted was the kitchen s base cabinets were not clean.

The Office of Child Care Licensing received a complaint on May 13, 2022, regarding the center possibly not having enough staff to sufficiently maintain ratios. An investigation was conducted on May 16, 2022. There is no evidence to substantiate that staff-to-child ratio were incorrect because children s and staff attendance records were reviewed and found to be compliant. A concern was noted because the attendance sheets did not accurately reflect the children s location after naptime. Tracking sheets must reflect where children are at all times. Administrator Savanna Hitch, confirmed that she did not properly fill out the attendance sheet for this day.
