On March 13, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding failure to call an ambulance for a potentially serious emergency, failure to provide a written incident report as required, not providing a top and bottom covering for a cot, failing to follow an action care plan, failing to keep child information confidential and disrespectful communication by staff with a parent. An investigation was conducted on March 24, 2025. This complaint is substantiated for failure to call an ambulance with suspicion of a seizure and not calling 911 as stated on Child #1 s action care plan. The administration team did not call for emergency response and did not follow the action plan on file for Child #1. This complaint is substantiated for failing to keep child information confidential. After hearing that a staff member was sharing confidential information to another parent, administration counseled and gave written disciplinary action to two staff regarding confidentiality. There is no evidence to substantiate this complaint for disrespectful communication by staff with a parent based on interviews of staff and administration, who all stated they spoke to the parent in a respectful manner. There is no evidence to substantiate this complaint for not providing a top and bottom covering for a cot. Child #1 was awake on their cot playing with the cot sheet when staff members helped Child #1 remove the cot sheet. There is no evidence to substantiate this complaint for not providing a written incident report. Parent #1 was provided an incident report and a ProCare report within one business day which Parent #1 refused to sign.

The Office of Child Care Licensing received a complaint on February 5, 2025, regarding allegations of failing to provide an injury report and disrespectful communication. An investigation was conducted on February 13, 2025. During the investigation interviews were held and statements were given. Based on interviews of staff and written statements provided, this complaint is substantiated for failing to provide an injury report and disrespectful communication. All staff interviewed stated there was no injury report provided and the communication between Staff #1 and Parent #1 was not professional. Staff #1 s employment was terminated on February 5, 2025, by the facility.

A licensee shall ensure an infant is given foods and drinks on demand or according to the infant s eating habits using the following guidelines: A parent or guardian provides a written feeding statement listing the foods and drinks, including specific formula or breast milk, an infant eats or drinks 12/12/24- corrected by staff disciplinary action and plan to monitor staff while feeding infants. Staff responsible were given disciplinary forms and there will be immediate change.

On January 27, 2022, OCCL received a complaint regarding failure to follow the procedures for handling food allergies. An investigation was conducted on February 1, 2022. This complaint is substantiated because a child was served food with a known allergen. A staff immediately removed the plate, once she noticed it was in front of the child.
