The Office of Child Care Licensing (OCCL) received a complaint on February 6, 2024, regarding concerns of inaccurate or altered attendance records, not following an open-door policy, staff members not completing orientation upon hire, hazardous conditions on the outdoor area, no soap and water in the classrooms, and a classroom operating out of square footage capacity. An investigation was conducted on February 21, 2024. This complaint is substantiated for operating a classroom out of square footage capacity because the two-year-old room had 12 children enrolled but the classroom measured for 10 children. This complaint is also substantiated for staff members not completing orientation immediately upon hire because two staff members did not complete the OCCL required Health and Safety training until over a month after their hire date. There is no evidence to substantiate this complaint for inaccurate or falsified attendance records because, upon review, all the child and staff attendance records were correct. While the Reporter did provide records with white-out on them, there is no evidence there was an attempt to falsify information. There is no evidence to substantiate not following the open-door policy as parents were observed coming into the center to drop off children and Owner Anne Ebaugh stated parents are welcome to enter at any time. There is no evidence to substantiate hazards on the playground based on observation of the playground. Some rust was observed on the chains attached to the swings, but it is out of the children s reach. It was recommended to the owner to clean or replace the whole chain. A dresser was observed up against the building, but this did not pose a safety hazard. The mulch on the playground, including in the fall zones, is at the required depth. There is no evidence to substantiate lack of soap and water in the classrooms or changing areas because soap and water bottles, disinfectant, and paper towels were present during the visit . Disposable covers were present for diaper changes.

Repair/replace the sink faucets in the back bathroom and vanity in the front bathroom to ensure cleanability. A center must have 35 square feet per child of usable floor space. The two-year-old classroom measured during the visit indicates a capacity of 10 children, not the enrolled 12. Send documentation to OCCL verifying the reduction of enrollment to 10 children by 3-8-24.
