On August 25, 2025, OCCL received a complaint regarding concerns of lack of supervision. A visit was conducted on August 29, 2025, to investigate this complaint. This complaint is substantiated for lack of supervision because during the investigation, Administrator #1 confirmed Child #1, who is two years old, had gotten out of the playground for an unknown period of time, and Staff #1 was unaware until Child #1 was returned by Pedestrian #1. An additional concern noted was Child #1 s parents and OCCL were not notified of the incident as required. In addition, according to attendance records Staff #1 was responsible for nine two-year-old children from 9:16 am to 12:00 pm, resulting in the classroom being out of staff-to-child ratio.

OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. On February 21, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of classrooms not being within the required temperature range and the heat not working, the use of portable space heaters, improper sanitation of cleaning of classroom items, and mixing of age groups due to low staffing. An investigation was conducted on February 27, 2025. This complaint is substantiated for using a portable space heater in one of the classrooms. Owner #1 stated they purchased a space heater to help with the cooler temperatures in the morning in the infant classroom. The cord to the space heater was not accessible to the children. There is no evidence to substantiate this complaint for the heat not working, as all classrooms were within an acceptable temperature range upon arrival. There is no evidence to substantiate this complaint for improper sanitation of classrooms. All classrooms had bleach and water and soap and water, and both products were available and accessible for staff upon arrival. Staff #1 stated staff cleans items after each activity, cleans tables after meals and at the end of the night, rugs are vacuumed, and floors are swept and mopped at the end of the day. There was no evidence to substantiate this complaint for the inappropriate mixing of age groups because there were no infants observed in the older classrooms. While there were three-year-old children mixed with school-age children, this occurred during the first 90 minutes of the day. An additional concern noted during this visit was the observation of improper staff-to-child ratios during the visit as well as verification of improper staff-to-child ratios on attendance records. According to the attendance records collected at the visit on February 25, 2025, Staff #4 had more than six and up to 14 children ages one to two alone from 7:40 am until 8:25 am. At 8 :25 am, seven children were signed out from the classroom, still leaving Staff #4 out of ratio with nine children until 9:30 am. Two additional children were signed out at 9:30 am, leaving Staff #4 out of ratio with seven children ages one and older the remainder of the day. Staff #4 was signed out for a break from 12:23 pm to 12:53 pm with no staff coverage. Additionally, the combined classroom with children ages three to school-age was observed over exceeding staff-to-child ratio during this visit with 12 children and one staff member. An additional concern noted was inaccurate child attendance records due to children not being signed out on multiple days of attendance records viewed.

The Office of Child Care Licensing (OCCL) received a complaint on March 13, 2023, regarding staff not following notification and documentation requirements regarding a child accident or injury. An investigation was conducted on March 22, 2023. This complaint is substantiated because the staff interviewed could not verify if any staff member notified a child s parent after the child got a scratch on his forehead at the center. Also, the staff could not confirm if anyone completed an injury report. The staff interviewed who were present when the incident happened admitted they did not complete a report for the child s injury to his forehead. An injury report for the scratch on the child s forehead was not available for review.
