A complaint was received on July 21, 2021, alleging multiple children have been hurt while being kept in the gym during the pandemic and there were many reports made due to an unsafe environment. Students were kept in the gym during the pandemic with missing ceiling tiles and large equipment to trip over that was not put away, there was water left on the floor, and trash cans did not have lids. The reporter stated a child was sleeping under a table and the children used a small trampoline in the gym. An investigation was conducted on July 27, 2021, and August 4, 2021. This complaint is substantiated for having an unsafe environment because there was a volleyball net and/or its poles/bases in an area that was accessible to children and resulted in a child tripping and falling. Additionally, there were areas of the gym floor that had water puddles that were not always promptly cleaned up, there were trash cans without lids, a trampoline was used by children in care, and two children were observed sleeping close to one another on the gym floor under a classroom table. Additionally, loose/dislodged ceiling tiles in the gym were observed during this visit and the gym is being used for indoor gross motor activities.

Ensure Radon Results on file are current. The current results expired on 12/4/2024. The administrator reported that the test samples have been collected and being sent to the manufacturer for results.

Staff #3 needs CPR, First Aid. Staff #4 needs 2 hours of Professional Development, Staff #5 needs 1 Letter of Reference. Staff #6 needs 8 hours of Professional Development. Staff #7 needs 3 hours of Professional Development. Send to OCCL when complete.
Ensure all staff files are complete. Staff #1 needs to complete OCCL's Health and Safety course. Send completed certificate to OCCL by January 16, 2023. Staff #2 needs to complete OCCL's Health & Safety course, as well as have a completed Health Appraisal & TB risk assessment and results. Send to OCCL when completed.
Ensure Child Files are complete. Child #1,3,4, and 6 need blood lead screening results. Send to OCCL when complete.
