Sleeping accommodations. Pack n play in the main room is ripped. Please remove.
Ratios must be maintained. Upon LS Reyes arrival, one staff member was present with 11 children (five under two years old), only six were signed in. Please be sure all staff stay on location, and all children are signed in. This incident was due to a staff member not feeling well and was corrected during the visit.
