Ensure any household members who turn 18 is fingerprinted within 5 days. A household member turned 18 in August of 2024, but was not fingerprinted until March of 2025. This was corrected at the visit when the Provider provided OCCL with a copy of the household member's eligibility letter. Ensure any household member who turns 18 has an adult health appraisal with TB screening with 30 days. Obtain adult health appraisal with TB screening for 18 year old household member.
Ensure child files are complete. One child needs an updated health appraisal with immunizations. One child needs a monthly feeding statement.

Licensing Specialist observed a child a sleep in a bouncer, Another child was observed in a room off the kitchen behind a closed door in a pack and play asleep with a bottle and toys in the pack and play. There was no tracking for the child sleeping in this room.
Ensure child files are complete. Two children need a health appraisal, updated immunization record, and blood lead test results. One child needs an infant feeding schedule.
