The cubbies in Classroom 3 were observed to be overcrowded, and children's belongings were touching. This was discussed and corrected when the items were arranged to no longer touch. Ensure diapers are disposed of in a foot-activated trashcan that has been lined with a plastic bag. The diaper trashcan in Classroom 3 did not have a plastic liner and trash was observed in the trashcan. This was discussed and corrected when a plastic liner was added to the trashcan.
On June 16, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision. This was also self-reported to OCCL by the center. An investigation was conducted on June 23, 2026. The complaint is substantiated. The investigation determined that Child #1 was able to exit the playground through an unsecured gate and travel approximately two blocks from the facility before being located by a City of Milford employee. A concern noted during the investigation was determined that the outdoor play area had not been adequately inspected for safety hazards prior to the children entering the playground. As a result, the gate to the outdoor play area remained open, providing Child #1 with an opportunity to leave the secured outdoor play area without staff awareness.
