The Office of Child Care Licensing (OCCL) received a complaint on November 19, 2025, regarding not following the suspension/ expulsion policy, not following accident/ injury procedures, and not providing positive behavior supports. An investigation began on December 1, 2025, and concluded on December 4, 2025. There is no evidence to substantiate this complaint because staff members interviewed stated that Child #1 would hit, scratch, and bite other children and staff members, and at times, was uncontrollable, which caused safety concerns to everyone, including Child #1. Also, staff stated they requested a copy of Child #1 s IEP from the parent several times, and the parent finally provided a copy on October 22, 2025. Additionally, the staff stated Child #1 never sustained any injury to his/her finger while in care.
Disposition: No Evidence to Substantiate
The Office of Child Care Licensing (OCCL) received a complaint on August 27, 2025, regarding improper discipline. An investigation began on September 3, 2025, and concluded on September 8, 2025. There is no evidence to substantiate this complaint because Staff #3 and Staff #4 stated that they do not know what caused Child #1 s injury to her arm. LS#1 reviewed over two hours of video footage from when Child #1 began eating lunch to when she got up from her nap and did not see an incident occur. Child #1 did not appear to be in distress when she laid down, or when she laid back down from going to the bathroom after nap. Child #1 was laying on her cot and then sat up and started crying and pointing at her arm when nap was almost over.
Disposition: No Evidence to Substantiate
The Office of Child Care Licensing (OCCL) received a complaint on August 6, 2024, regarding an unqualified staff member alone with children and a lack of positive behavior support from staff. An investigation was conducted on August 7, 2024. This complaint is substantiated for an unqualified staff member alone with children because Staff #1 admitted to being alone with children before obtaining a school-age intern certification on August 5, 2024. Additionally, another staff member stated Staff #1 has been alone with children. There is no evidence to substantiate that Staff #1 had been aggressive and demeaning. Staff members and children interviewed stated staff had been firm, but not aggressive.

The Office of Child Care Licensing received a complaint on May 7, 2024, regarding lack of supervision. An investigation was conducted on May 7, 2024. This complaint is substantiated for lack of supervision because Staff #1 was observed on video footage supervising the children in the bathroom, she then left to place a pair of shoes in a cubby and to close the classroom blinds. During this investigation, a concern noted was Staff #2 was observed in video footage roughly handling a child.
