The Office of Child Care Licensing (OCCL) received a complaint on June 13, 2023, regarding staff-to-child ratio, not changing soiled clothing, not meeting the needs of the child, and not following proper health exclusions. An investigation was conducted on June 22, 2023. This complaint is substantiated for staff-to-child ratio because an intern was observed alone with infants. Also, according to attendance records, Ms. Diaz was signed out of the infant classroom before all of infants left on June 6, 2023, June 9, 2023, June 13, 2023, June 15, 2023, and June 20, 2023, and no other staff were signed in. There is no evidence to substantiate the allegations of not changing soiled clothing, not meeting the needs of the child, and not following proper health exclusions because staff denied these allegations, and this was not observed at the time of this visit. An additional concern noted during the visit was staff attendance was not accurate.

OCCL received a complaint on August 4, 2022, regarding failing to follow accident and injury procedures, not serving milk with lunch, not following their handbook for health exclusions, lack of supervision, not having enough equipment, not following screen time requirements, and having people in the building who should not be there. An investigation was conducted on August 17, 2022. This complaint is substantiated for lack of supervision because a picture was submitted showing Staff #2 asleep in the classroom and Ms. Judy stated she probably sleeps in the classroom, but she has never observed it. It is substantiated for not serving milk with lunch because when Specialist Rasin-Benn checked the fridge after lunch, there was not any milk and Ms. Judy stated they ran out in the morning. It is substantiated for not following screen time requirements because Ms. Judy stated two children bring their video games from home and play them during naptime instead of sleeping. There is no evidence to substantiate the allegations of not following accident and injury procedures, not following their handbook for health exclusions, not having enough equipment, and having people in the building who should not be there because Ms. Judy denied these allegations and Specialist Rasin-Benn observed adequate equipment in the classrooms. An additional concern noted was attendance records were not correct because a staff member had not signed in.

Medication must be current. An epi pen was out dated. This was corrected when the provider gave the pen back to the parent.
Ratios must be maintained at all times. Administrator Judy Diaz was in the infant classroom with five (5) infants. This was corrected when an additional staff member entered the classroom.

Ensure interns are not alone with infants upon arrival,. Judy opened the door and went to her office with the Licensing Supervisor. They then walked into the infant classroom where Ms. Burton (an intern) was alone with three infants. This was discussed and corrected when Judy entered the classroom. Ensure all staff are practicing safe sleep procedures. LS Smith observed one infant sleep in the crib with a bib around its neck.
Ensure all infants have an updated feeding schedule completed by the guardian monthly. Ensure bottles are labeled and stored properly. Ensure breast milk and formula is stored correctly in the fridge at the proper temperature. Mixing prepared formula or breast milk in a bottle with anything else requires written permission from an infant s health care provider. LS Smith observed cereal in an infants bottle.
Ensure ratios are maintained at all times. Staff #1, who is an aide, was observed alone in the classroom with four infants. This was corrected when a qualified teacher entered the classroom.
Ensure lesson plans a re posted and have all the required components. Infant room did not have a lesson plan. Pre-school room's lesson plan did not have all the required components.

OCCL received a complaint on June 1, 2022, regarding improper staff-to-child ratios, failure to use security procedures, and the air conditioner not working. An investigation was conducted on June 2, 2022. This complaint is substantiated for improper staff-to-child ratios and failure to use security procedures because there was an unqualified staff supervising two separate classrooms, the front door was unlocked, and there was not a staff member monitoring the front door to ensure security procedures were followed. There is no evidence to substantiate the allegation of the air conditioner not working because staff denied this and the temperature was under the maximum allowed temperature. Additional concerns noted were staff attendance records were not accurate, children were missing top or bottom coverings for naps, and an infant was observed awake in a crib with a bottle.

OCCL received a complaint on June 9, 2022, regarding the owner showing up late to open the building, the owner s daughter running things when the owner is unavailable, and a staff member having her grandson at the center chilling in the classroom. An investigation was conducted on June 12, 2022. This complaint is substantiated because Ms. Diaz stated she does show up late to the center in the morning. There is no evidence to substantiate the allegations of the owner s daughter running things or a staff member having her grandson at the center chilling in the classroom. Both of these events did happen, but after the investigation it was determined these are not regulation violations.
