On June 15, 2022, a complaint was accepted for investigation by OCCL regarding the center not following age-appropriate serving sizes for meals. An investigation was conducted on June 27, 2022. There is no evidence to substantiate this complaint because Ms. Cannon and Staff #1 stated children are served the appropriate amount during meals and seconds are given if asked for by a child. Also, LS Donovan observed a meal served with appropriate serving sizes.

There is no evidence to substantiate this complaint because Ms. Cannon stated parents are notified of accidents or injuries via the Bright Wheels app and then sent an electronic accident or injury report and seven reports signed by a parent or guardian were observed for June 14, 2022.

OCCL received a complaint on February 24, 2022 regarding improper staff-to-child ratio, staff and children not properly wearing face coverings or at all, and not following diaper requirements. An investigation was conducted on February 24, 2022. This complaint is no evidence to substantiate because all staff interviewed denied these allegations, proper staff-to-child ratio was maintained during the visit and according to reviewed attendance, LS Sommer observed face coverings worn properly by all staff, and observed documentation of diaper changes which met diapering requirements for child care facilities.

OCCL received a complaint on December 14, 2021, regarding improper discipline and improper staff-to-child ratio. An investigation was conducted on December 20, 2021 and concluded January 12, 2022. There is no evidence to substantiate this complaint for improper discipline for screaming at children or withholding food as punishment because all staff interviewed denied the allegations. There is no evidence to substantiate for improper staff-to-child ratio because staff-to-child ratio throughout the facility was maintained through qualified staff, and interviewed staff were aware of proper staff-to-child ratio and denied allegations regarding improper staff-to-child ratio. There is no evidence to substantiate this complaint for failing to provide physical care routines because the center obtained documentation from Parent #1 during enrollment regarding Child #1 s physical care routines on a document titled getting to know your child.

An investigation was conducted on September 16, 2021. There is no evidence to substantiate this complaint for not checking diapers hourly and not understanding the child s needs. All staff interviewed stated diapers are checked hourly, changed at least every two hours, and as needed. Children have not been denied using the bathroom because it is located in the classroom and staff stated they encourage children to freely use it. There is no evidence to substantiate this complaint for not notifying a parent of a head injury or completing an accident or injury report because Ms. Cannon stated she was unaware of the alleged incidents, all staff understood the accident or injury procedure, and LS Kenton observed completed accident or injury forms in the children s files.

OCCL received a complaint on September 2, 2021, regarding not following the COVID-19 protocols by allowing a sick parent into the building and failing to notify parents of a positive case of Covid-19. An investigation was conducted on September 16, 2021. There is no evidence to substantiate this complaint for not following protocols because Ms. Cannon stated the parent did not enter the building and the Division of Public Health was notified when Child #1 tested positive. This complaint is substantiated because according to attendance records, there were multiple occasions of unqualified staff alone with children and improper staff-to-child ratio. An additional concern noted is incorrect documentation of arrival and departure times on attendance records for both staff and children on multiple occasions.
