The Office of Child Care Licensing (OCCL) received a complaint regarding improper staff-to-child ratio, staff using alcohol or drugs that could adversely affect job functions, the administrator not being present the required time, lack of positive behavior supports, handwashing, and diapering procedures. An investigation began on February 18, 2026, and concluded on February 19, 2026. This complaint is substantiated because, according to the administrator s attendance record, Owner #1 was not present the required 27.5 hours per week. There is no evidence to substantiate the allegations of improper staff-to-child ratio, staff using alcohol or drugs that could adversely affect job functions, lack of positive behavior supports, handwashing, or diapering procedures based on interviews, observations, and a review of documentation. An additional concern noted during this investigation was children were observed lying on their sleeping equipment without top or bottom covers.

The Office of Child Care Licensing (OCCL) received a complaint on October 14, 2022, regarding improper staff-to-child ratios and security procedures. An investigation was conducted on October 21, 2022. This complaint is substantiated for not following security procedures because a staff member stated the center door is propped when they go outside to ensure re-entry into the building. There is no evidence to substantiate the allegation of improper staff-to-child ratio because proper staff-to-child ratios were observed during the visit, and attendance records from October 10, 2022, to October 14, 2022, reflected proper staff-to-child ratios. A concern noted was staff were not signed out on some attendance records.

OCCL received a complaint on July 14, 2022, regarding improper staff-to-child ratios. An investigation began on July 26, 2022, and concluded on August 8, 2022. This complaint is substantiated because according to attendance records, on July 1, 2022, July 5, 2022, July 6, 2022, July 7, 2022, July 8, 2022, and July 11, 2022, Staff #1, an aide, was alone with children in the one-year-old and two-year-old classroom. Also, during the visit, two staff members who are aides were alone with five children. Additional concerns noted were attendance records for staff and children were not accurately documented on several occasions.

On October 5, 2021, a complaint was accepted for investigation by OCCL regarding lack of supervision and not ensuring non-prescription medication is stored securely out of reach of children. LS Donovan and McPhatter investigated the complaint from October 7, 2021, to October 13, 2021. This complaint is substantiated for storing non-prescription medication in the first aid fanny pack where a child gained access and swallowed an antacid. There is no evidence to substantiate this complaint for lack of supervision because the staff was present on the playground caring for an injured child while Child #1 ingested the antacid pill. Concerns noted were a child received an injury while on the playground and the center did not document the injury and the center failed to notify OCCL verbally within one business day a child receiving medical treatment due to an injury at the center.
