On July 14, 2022, a complaint was accepted for investigation by the Institutional Abuse Unit (IA) and the Office of Child Care Licensing (OCCL). On January 30, 2023, IA informed OCCL the complaint will be closed as unsubstantiated with concern. The concern noted was how Administrator Ondrea Daniels carried Child #1 out of the classroom. OCCL conducted an investigation regarding improper discipline on February 9, 2023, by reviewing the video footage of the incident. This complaint is substantiated for improper disciple because the video footage showed Ms. Daniels holding Child #1's arms over his head while she was pulling and dragging him out of the classroom.

On July 26, 2022, a complaint was accepted for investigation by OCCL regarding a lack of supervision. An investigation began on June 26, 2022, and concluded on July 27, 2022. There is no evidence to substantiate this complaint because staff members and children interviewed stated children are supervised at all times.
Disposition: No Evidence to Substantiate
On January 4, 2022 and January 12, 2022, a complaint was accepted for investigation by OCCL regarding not allowing officials from Institutional Abuse Unit and other agencies access to information for an investigation to determine compliance with applicable regulations or laws. The complaint was also accepted for failing to notify parent of Child #1 s behavior problem. This complaint is substantiated because the complainant had to obtain a subpoena from the DOJ because Mr. Toner was reluctant to cooperate with the investigation and to provide the needed information. There is no evidence to substantiate the allegations of the center not addressing and notifying the parent of Child #1 s behavior issues because the center stated the parent was informed. The center provided child observation notes, disciplinary reports that were signed by the child s parent, and a structural chart created by the staff to help child understand the behavior expectations at the center.

Ensure direct supervision of all children in care. Upon arrival at the center four children was observed leaving the Terrific Tigers, Two children entered the Lightning Lion, no staff was in the lobby or the Lion room. Two children entered the Lucky Lynx with no staff. This was corrected when a staff brought the children back to the Tiger classroom. Later, another child left the Tiger room and entered the lobby to go to a classroom with no staff present.. Another time a staff allow children to go back and forth from the Lynx room and the Tiger room. A conversation was held with staff about supervising children in the door threshold was not allowed.
