Ensure each staff document their exact arrival and departure times when working in the classroom. Staff members left the classroom and return without signing in or out of the classrooms. (This was corrected when staff members agreed)

OCCL investigations of regulatory non- compliance begin when clearance from other agencies, when applicable, has been received. The Office of Child Care Licensing (OCCL) received a self-reported complaint on November 18, 2024, regarding improper discipline. An investigation began on November 18, 2024, and concluded on January 8, 2025. This complaint is substantiated for improper discipline because Staff #1 was observed roughly handling Child #1 in the classroom on video footage as reported by the administrator. Staff #1 s employment was terminated by the center after they reviewed the video footage.

The Office of Child Care Licensing received a complaint on August 31, 2023, regarding using material used for cleaning on a child s face and not maintaining safety. An investigation was conducted on September 5, 2023. This complaint is substantiated because Ms. Ling and Mr. Toner stated Staff #1 used a generic magic eraser on Child #1's face when he wrote on his face with a marker. Staff #1 s employment was terminated due to the incident. Ms. Ling also reported this incident to OCCL on August 29, 2023.

Ensure outside gates are self- closing. Repair the SA gate.
Ensure diaper only can is not used for trash. Remove trash fromt he diaper can in room 16-17. Place a liner in the diaper only can in room 18.
Ensure MAR are completed for medication. Child #8 has one MAR for two different diaper cream. Provide separate MAR for both diaper cream.
Ensure child file are completed. Child #9 need an updated health appraisal with immunization.
Ensure walls are clean and in good repair. Room 21 bathroom light cover needs repair, Room 20 paint on back of door is peeling, bathroom door tape residue and paint peeling, intake vent paint above peeling, Room 18 cabinets and shelf have tape residue, door frame of bathroom paint peeling, Room 15 paint peeling and door, right of paper towel dispenser needs holes filled and paint and tape residue on doors. Repair painted areas filled holes and paint. Ensure room 22/23 and 24 are operable, repair walls that have paint peeling, room 19 repair base board under the window,
Ensure walls are in good repair, Room 15, the block area has paint peeling. Room 18 circle time area has paint peeling, Room 20 paint peeling in block area.
Ensure no prescription medication is labeled with the child's name. Six OTC medication do not have a child's name on them.
Ensure a comprehensive background check is completed with eligibility letter on file for staff. Staff #1 needs a comprehensive background check and eligibility letter.
Ensure direct supervision occurs at all times. A child pre K child was observed in the two-year-old bathroom while the staff member were in the multipurpose room for a presentation. This was corrected when the child was brought to the multipurpose room with the staff.
