On March 14, 2025, Child #1 was left unsupervised for four minutes on the playground. Staff #1 and Staff #2 were retrained on name-to-face policy regarding supervision.
On March 28, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding not following positive behavior supports, improper interactions with toddlers by placing a toddler in confining equipment for more than 30 minutes and failing to follow their suspension and expulsion policy. This complaint is substantiated for improper interactions with toddlers. As viewed on video footage, Child #1 was confined for longer than 30 minutes in a highchair. There is no evidence to substantiate the complaint for not using positive behavior supports. The administrators and staff members report using several strategies to support Child #1 and the family through communication, obtaining behavioral intervention with support from Person #1, and implementing recommended ideas and activities in the classroom. There is no evidence to substantiate this complaint for failing to follow their suspension and expulsion policy. After reviewing the center s family handbook, the administrators followed their policy regarding suspension or disenrollment which states in certain circumstances it may be necessary to discontinue a child s enrollment. Every effort will be made to correct a situation before a final decision is made and we reserve the right to end the enrollment of a child at any time for any reason deemed appropriate.

On April 14, 2023, a complaint was called into the Report Line and accepted for investigation by the Institutional Abuse Unit (IAU). It was sent to the Office of Child Care Licensing (OCCL) on April 14, 2023, and was accepted for investigation for improper discipline. On September 29, 2023, the IAU investigator notified OCCL their investigation was completed as no evidence to substantiate with concern. OCCL will substantiate this complaint for improper discipline because the IAU investigator stated the video footage reviewed showed Staff #1 used improper discipline with Child #1. Administrator Randi Racan, Staff #2, and Staff #3 all stated they observed Staff #1 use improper discipline with Child #1. Center administration had already terminated Staff #1 s employment.

Ensure all staff files are complete. Staff #1 needs a health appraisal with TB. Staff #2, #3, #4, and #5 need a documented 2nd attempt for Service Letters.
Ensure only qualified staff are supervising children. An unqualified staff member was alone left alone with eight in the preschool classroom and one unqualified staff was left alone in the Pre-K classroom with ten children. Staff #1 and Staff #2 need a DEEDS certificate or proof documents have been submitted to DEEDS and a plan must be developed to ensure qualified staff are directly supervising children at all times.
