The Office of Child Care Licensing (OCCL) received a complaint on, August 8, 2024, regarding, improper staff-to-child ratio, interns alone with infants, positive behavior supports, and adult health requirements. An investigation was conducted on August 15, 2024. This complaint is substantiated for interns being alone with infants because according to attendance records, Staff #4, an intern, was alone with infants on July 15, 2024, from 12:02 pm until 1:02 pm, on July 23, 2024, from 7:48 am until 8:19 am, and from 11:41 am to 1:00 pm. There is no evidence to substantiate the other allegations based on staff interviews and a review of attendance records.

The Office of Child Care Licensing (OCCL) received a complaint on February 20, 2024, regarding the center giving out or allowing the use of a child s or family s information without written permission from the parent or guardian. An investigation was conducted on February 22, 2024. This complaint is substantiated because the owner stated the allegations did occur. A concern noted was during the visit the licensing specialists observed a child run from the restroom back to the classroom. When asked, the owner stated Staff #2 was supervising the child going to the bathroom, and the owner was present in the school-age classroom speaking with the licensing specialists when the child exited the bathroom and ran to his classroom.

OCCL received a complaint on August 26, 2022, regarding a staff member telling a child to hit another child back, which resulted in the children fighting one another and staff having to break the fight up. An investigation was conducted on August 31, 2022. This complaint is substantiated because the administrator stated Staff #1 told Child #2 to hit Child #1 back.

OCCL received a complaint on July 13, 2021, regarding screen time activities not being age-appropriate. An investigation was conducted on July 14, 2021. The complaint is substantiated for screen time activities not being age-appropriate because although parent permission was obtained, Administrator Johnson admitted to taking a group of children to the movie theater to see Black Widow, which is rated PG-13. PG-13 movies are not age-appropriate for children ages four - school-age.

Post the diaper changing procedure in the infant classroom. This was corrected at the visit.
Staff must be finger printed prior to starting at the center. Have Staff #2 fingerprinted. This was corrected at the visit when Ms. Johnson provided a fingerprint receipt for the employee.
Purchase additional water for sheltering in place. Send receipt to OCCL.

Replenish the fall zones under the ship outdoor play area. Ms. Johnson stated she is having pour and play installed and is on the schedule. Send photos to OCCL.
Purchase foot-activated diaper pails for the boys and girls bathroom. Send photos to OCCL.

Ensure floors walls, carpets, and ceilings are in good repair. The carpet in Huggable Hills needs to be flattened out. Repair the broken floor boards and missing thresholds in Little Learners and Big Learners classrooms. Repair the floor board between the boys and girls bathroom. Repair the broken ceiling tile in the Little Learners classroom. Send photos to OCCL.
