Staff A was hired on 11/18/2024 and was working in the classroom the day of this visit. This licensor was emailed a copy of the orientation template after the visit and it reflected that Staff A completed orientation on 09/17/2025 which was after this licensing visit. It is required that staff complete orientation when they are hired and are not to be alone with children until they complete orientation. This is a repeat violation that was cited on 09/17/2024.
Staff B and staff C were hired in January of 2025 and letters were sent notifying the program that Staff B and C need to complete fingerprinting through the Child Development Division. Staff B was fingerprinted after the licensing visit and Staff C was fingerprinted in August of 2025. This is a repeat violation that was cited during the licensing visit on 09/17/2024.
BFIS account for substitute Staff A shows they have not completed the fingeprinting process. Staff A began working for the program on 1/14/22. A letter advising Staff A of this requirement was generated and issued on 1/14/22. Review of Associated Parties list during this visit confirmed Staff A is still employed with the program.
Review of all staff BFIS credential accounts determined there are twelve (12) staff identified by the program as being counted in ratio and/or left alone with children that have not completed this required training. This is a repeat violation.

A review of Staff BFIS accounts reflected that Staff B, C, and D had not yet completed an approved Orientation Training. School Personnel confirmed that the listed staff had not yet completed an approved Orientation Training. All staff listed have been employed with the program for more than 1 year.

Review of BFIS determined eleven (11) staff identified by the program as being counted in ratio and/or left alone with children, have not completed this required training. This is a repeat violation.
Review of BFIS Quality and Credential accounts revealed six (6) staff members accounts do not contain the required documentation as outlined by this rule. All six (6) staff have been employed with the program for more than six (6) months. This is a repeat violation.
Staff files were determined to be maintained off site at the Supervisory Union office. Observation during stie visit as well as post site visit follow up determined program does not have an updated affidavit on site as required.
Based on review of staff credential accounts in BFIS, it is determined that twelve (12) staff identified by the program as being counted in ratios and/or left alone with children, who have not completed or are not current with this required training. All twelve (12) staff have been employed with the program for more than three (3) months.This is a repeat violation.
A review of Staff BFIS accounts reflected that Staff B, C, D, and E had not yet competed the required Mandated Reporter training. These staff members include Substitutes, an Aide, and an Auxiliary Staff. The program director confirmed that while these staff members did complete a school wide mandated reporter training, they had not yet completed the Mandated Reporter training; Protecting Vermont's Children: Reporting Abuse and Neglect, which is required by regulation. All staff listed have been employed by the program for more than 1 year.
A review of the Staff F's BFIS Quality and Credential account reflected that Staff F had not yet completed an approved Emergency Preparedness Training. School Personnel confirmed that the Staff F is considered the representative of the licensee and has been in that position for more than 365 days.
A review of Staff BFIS accounts reflected that Staff A, B, C, and D, did not have current pediatric first aid and infant and child CPR certifications. School Personnel confirmed that these staff members did not have current first aid and CPR certifications. All listed Staff have been employed with the program for more than 3 months.
Staff BFIS Quality and Credential accounts for Staff A, B, C, D, and G did not contain verification of qualifications. School Personnel confirmed that documentation of qualifications for these staff members had not yet been submitted to Northern Lights at CCV. All staff listed have been employed with the program for more than 1 year. As a result, qualifications of assistants, aides, and substitutes were unable to be assessed.
