During the visit, Staff B and Staff C were observed to be left alone with groups of children on the playground. Neither Staff B or Staff C had received a final clearance letter from the Division based on a fingerprint supported background check.

A review of staff files and staff BFIS accounts reflected that Staff D had not completed at least four hours of annual professional development. Staff D has worked with the program for more than 12 months.
A review of staff files and BFIS credential accounts reflected that Staff A did not have documentation of qualifications for their position. As of 07/24/2025, the program was not able to make documentation of Staff A s Qualifications as Program Staff available for review.
Review of BFIS accounts for Staff A and Staff B reveal both staff have yet to complete the fingerpritning process. Staff A began their employment with the program on 8/21/22 and was issued a letter advising them of fingerprinting requirement on 9/8/22. Staff B has been employed with the program since 9/19/22 and was issued a letter advising them of fingerprinting requirement on 9/29/22.
Review of BFIS Credential accounts, as well site visit follow up email exchange with program determine Staff A and Staff B have not yet completed the training required by this rule. Staff A began their employment with the program on 9/19/22 and Staff B begain their employment with the program on 9/8/22.
Staff B began their employment with the program on 9/8/22. Review of credential accounts for both Staff B, as well as follow up email correspondance with the program, determined Staff B has not obtained their CPR and First Aid certification trainings.
A review of staff BFIS quality and credential account and staff files reflected that Staff A and Staff B were not yet certified in infant and child CPR and pediatric first. The Director confirmed that Staff A and Staff B had not yet become certified in CPR and first aid. Staff A and Staff B have worked with the program for more than 3 months. Staff A and Staff B were present and working with children during the visit. This is a repeat violation.
During the visit, 3 one gallon containers of bleach were observed to be on the counter near the children s hand washing sink in the main classroom, a spray bottle of blue disinfectant was on a table used for eating in the main classroom, a bottle of children s ibuprophen was in an unlocked cabinet, and a bottle of children s ibuprophen was in each of the 3 outdoor "Go Bag"s which were on the floor in the main classroom. All of these items were at children s reach and accessible to children. Further, Staff A stated that each staff member takes a "Go Bag" when taking children outside and the bags are often placed on a bench or on the ground in the areas where the children play outside.
Staff A stated that evacuation drills had been conducted 2 times since January 2021. The evacuation drill log reflected that no evacuation drills had been conducted since January 2021. This is a repeat violation. A violation for this rule was also cited on 09/25/2019.
Staff A stated that while the staff typically have the same schedule each week, there is not a written schedule of assignments for staff which includes staff absences or the use of subsititutes within the program. This is a repeat violation. A violation to this rule was also cited on 04/05/2017.
The children did not wash their hands after outside play. The children were asked to wash their hands prior to eating snack, however, 7 children were observed to wash their hands and 15 children were observed to eat snack.
Tara stated that the she does not have an evacuation drill log for the current and previous year on site for the program. Tara did produce a paper stating that an evacuation drill had been performed in August, but it did not include required information such as the number of children and staff who were present and how long it took to perform the drill.
A review of staff files revealed that one staff member did not have CPR and first aid certification documentation and one staff had documentation of certification documentation for CPR, but not for first aid.
While there is a record of hours staff worked there is not a record of staff assignments.
While the mulch under the swings appeared to have an adequate depth, it did not extend out twice the length of the top bar in the front and back of the swing set. Children were observed jumping off of the swings and landing beyond the mulch that was present.
