The children's files were audited, and the required well care exam was missing for two (2) children who have been in care for more than 45 days.
After discussion with the program director and review of the BFIS Quality and Credential account it was found that staff had missing information in their BFIS Quality and credential account.
Upon review of staff BFIS Quality and Credential accounts and discussion with the program director, it was found that staff had missing or incomplete documentation in their accounts. Staff B, C, G Quality and Credential accounts are missing CPR/First Aid certification. Staff G Quality and Credential account is missing the Vermont Orientation training certification. Staff A, D, E, F, H, I, J, K, and L Quality and Credential accounts are missing the Mandated Reporter training certification. Staff B is missing educational documentation. All staff have been in the program for more than 6 months.
