A staff member who had not completed the fingerprint process brought a child into the building alone to use the bathroom. The other staff stayed outside on the playground.

A review of the staff's BFIS Quality and Credential account and review of staff files confirmed three (3) staff members did not complete the VT Orientation training. The three staff members have been working in the program for longer than one (1) month. It is required for staff to complete the VT Orientation within one (1) month of their first day working with children.
A staff member who has worked in the program since November 2024 did not complete the fingerprint process for the Child Development Division. The fingerprint letter from CDD was issued on 11/6/2024 requesting this staff member to complete the fingerprint process.
A review of the staff's BFIS Quality and Credential account, and a review of the staff's file, and verbal confirmation, it was determined that the staff member was missing seven (7) hours of professional development between 9/23/2023 to 9/23/2024.

The director stated the Parent Notification Letter, that was issued by the Division on 8/3/2023, due to a serious violation cited, was not mailed, or e-mailed to parents as directed.
The preschool classroom was operating with eleven (11) children and one (1) teacher the day of this licensing visit. The second teacher was not scheduled to arrive till 11:00 and the Licensor arrived at 10:00. The teacher knew they had eleven (11) children but did not fix the problem until the Licensor arrived and noted them out of compliance. Operating out of staff/child ratio is a serious violation.
On August 15, 16 and 17 the attendance reflects the program to have twenty-one (21) to twenty-two (22) children in care. Staff stated they had a family who needed emergency care on two (2) days and a child arrived who was not scheduled for care on Wednesday August 17. The program allowed the children to stay even though they were at their maximum capacity of twenty (20) children. This violation was cited previously on 6/18/2019.
The Children's files were audited, and the required immunization record was missing for two (2) children.
After a review of the staff's BFIS quality and credential accounts, it was determined that four (4) staff employed longer than six (6) months did not have an up to date BFIS quality and credential account as required and was missing qualification information and required training certificates.
The program had 23 children in care during a licensing visit on 6/18/2019 and the total capacity of children allowed in the program at one time is 18.

The Children's files were audited, and the required well care exam was missing for three (3) children who have been in care for more than 45 days.
The diaper changing procedure was not posted near the diaper changing area in the infant classroom.
