7.4.4 · All staff, with the exception of substitutes, shall complete fifteen (15) clock hours of annual professional development activities as required in the rule 7.4.2 of these regulations.

It was found that the provider completed eight (8) of the required fifteen (15) hours of annual professional development for the period of 9/25/2023 to 9/24/2024. The provider said that she believed she was in compliance based on information received during the last compliance visit, and as a result, did not complete the remaining seven (7) hours.

6.2.1.5 · The registered FCCP shall ensure that the number of staff present meets requirements described in the chart below for ages of children, maximum group size, and staff/child ratios at any time of day.

The provider acknowledged that three (3) children under the age of two were present at the program at the same time, and that additional children were also present on some of those days. The rule states that a provider may only have two (2) children under the age of two (2) unless there is an assistant working with the provider or there are no other children present at the same time. This is a serious violation.
