Staff A stated there was not a staff schedule on-site, and that staffing patterns have fluctuated due to coverage issues. The childrens attendance lists staff that are assigned; however this staffing pattern does not show the dates, or times present, and is not what was observed the day of the visit.

Upon arrival at the program at 4:30 there were eight (8) children present, but ten (10) children reflected on the attendance sheet. At 4:50 after some children left, nine (9) children were reflected on the attendance, but only four (4) children were in care. When mentioned to Staff A, they advised that the attendance had not been updated at the time children left. Upon request to see previous attendance records, Staff A stated they did not have access to the records.
Five (5) staff have not maintained CPR/First Aid certification as required.
