Upon review of the Provider's BFIS account it was identified that during the licensing renewal process in 2023, the provider was required to submit to water testing as required by this rule. The provider confirmed via email that such testing had not been conducted since 2017.
Upon review of information from the Vermont Department of Health it was identified that this Provider has not tested for lead in drinking water since 2019. Based on Vermont standards testing was required to be conducted in 2022. Provider statements reflect they did have correspondence with the Vermont Department of Health and received a lead water testing kit, but that samples have not yet been drawn or sent to a laboratory.

While the Provider was serving snack and while children were eating there were dogs in that space and remained in that space for the duration of snack. One dog was observed lying under the table and putting their nose up to the tables edge.
