A review of the BFIS quality and credential account and statement by provider during this visit confirms that seven (7) of the required fifteen (15) hours of professional development training required between 4/28/2024 - 4/28/2025 was completed.

During the visit, the provider stated that she does not always remember to conduct and/or record monthly evacuation drills and was not able to produce an up-to-date evacuation drill log. This does not meet the requirements of this rule.
During the visit, it was noted that the fire extinguisher intended for the program had a tag which expired in August of 2023.
Review of BFIS and provider's files on site, it is determined only twenty-three (23) of the required thirty (30) hours of professional development was completed for the 2021/2022 year. Based on site visit report from the 1/31/22 visit it was determined provider had not completed any professional development hours for the 2020/2021 year. Based on this provider was to complete a total of thirty (30) hours for the 2021/2022 year.
No documentation was available to show professional development from September 2020 to September 2021, as required. The provider stated that they may have completed one training, but did not have documentation to confirm. This is a violation because it is required for the FCCH provider to complete 15 hours of professional development each year.
The FCCH provider confirmed that their CPR and First Aid certifications were expired. This is a violation because the CPR and First Aid certifications must be maintained.
Tina reports that evacuation drills are practiced once a month; however, she did not have documentation of evacuations for 2019.
