The FCCP shall submit a Records Check Authorization form to the Division for any new individuals, sixteen (16) years of age or older, residing in the FCCH or present more than five (5) times in 365 days during the FCCH's hours of operation as required in the rule 2.3.7.2 of these regulations.

A record check authorization form was not submitted to the Division for a household member who turned sixteen (16) in December 2020.

7.2.2 · Not corrected

All staff, with the exception of substitutes, shall complete fifteen (15) clock hours of annual professional development activities as required in the rule 7.4.2 of these regulations.

A review of BFIS and statements made by the provider confirmed that the provider had not completed the required fifteen (15) hours of annual professional development for the year 9/16/2020 to 9/16/2021. The provider said that there wasn't any in-person professional development offered, and that online professional development does not work well for her.

7.4.4 · Not corrected

Within six (6) months of initial date of employment, the FCCP and all staff shall maintain an up-to-date BFIS Quality and Credential Account. Documentation, verification of qualifications, and all annual professional development activities as specified in the rules sections 7.3 and 7.4 of these regulations shall be submitted to NLCDC to be verified and maintained in BFIS.

After review of staff Quality and Credential Accounts, it was determined that 2 (two) staff are missing required qualification documentation in the BFIS Quality and Credential Accounts. This is a violation because both staff have been employed longer than six (6) months and it is required to have proof of high school diploma or GED in the BFIS Quality and Credential Account.

3.3.7.3 · Not corrected

The FCCP shall ensure that menus are planned in advance, dated, and posted so that they are clearly visible to parents. Any changes to the posted menu shall be noted. Menus of meals and snacks for the preceding six (6) weeks shall be kept on file.

It was determined that the program provides meals and snacks but does not preplan, post, or keep record of meals and snacks. This is a violation because it is required to preplan, post, and keep 6 (six) weeks of menus.

5.11.1.3 · Not corrected

The FCCP shall ensure that each child, except school age children, has an individual crib, port-a-crib, cot, mat, or bed that is safe for the child's age and development, in good repair, and used by only one child each day. No child shall nap directly on the floor or on furniture not designed for sleeping. Cribs or port-a-cribs shall be used for children under the age of twelve (12) months. Children shall not nap in cribs or port-a-cribs if they are physically able to climb out unassisted.

It was observed that the program utilized three large nonporous mats during nap for all the children. Two (2) of the nonporous nap mats had two (2) children on each. This is a violation because each child should have an individual nonporous nap mat.
