The program terminated care for a child enrolled at the program, effective immediately, without giving the required five (5) days notice.
The provider shared health information regarding a child in care, without permission, with other families in a group text message. This is a breach of confidentiality.

The BFIS accounts for associated parties were not up to date. The accounts for the provider and an assistant do not have documentation of education needed to assess qualifications. The BFIS account for the assistant is also missing credentials and annual professional development.
Wendy did not notify a parent that a 9 month old child had been injured until approximately 2.5 hours after the injury occurred on 5/3/2019. Wendy confirmed that at approximately 11AM Child A, who had pulled herself to a standing position, fell over onto a doll stroller. She shared that the child had a very small abrasion on her head, and a short time later seemed uncharacteristically fussy and cried when crawling. Wendy shared that the child seemed to favor one arm and was fussing when crawling after she woke from a nap. Wendy gave the child Tylenol at 1:07PM and contacted the parent at approximately 1:30PM. It was later determined that the child had a broken clavicle. This rule requires that the provider notify a parent as quickly as possible after an accident or injury occurs.
