Evidence: Between 8:30 am - 8:45 am, five of eight classrooms (groupings of children) did not have a staff member qualified as program leader or program director. Staff records were reviewed with staff member in charge and confirmed there was not sufficient education or training hours for the individuals to be qualified.

Evidence: On approximately June 14, 2024 during the morning hours, 2 staff were present with 11 children in the toddler classroom for approximately 20 minutes. Child #1 (age 14 months) was the youngest in the group.
Evidence: The following was found during record review: 1. The record for staff #2 (start date 2/14/2024) had a TB test/screening dated 3/7/2024. 2. The record for staff #5 (start date 2/26/2024) had a TB test/screening dated 2/29/2024.
Evidence: On the outside playground, 1 of 2 chains was broken on the bottom portion of a green triangle climbing piece that anchors the triangle to the ground.

Evidence: On October 20, 2023, between approximately 2:30 pm ? 3:00 pm, staff #1 was observed roughly handling child #1 (16 months old). Child #1 was crawling off her cot and staff #1 pulled child by her ankles approximately 16? back onto her cot. Staff #1 was then observed picking child #1 up by the outside of her upper arms and placing her on the changing table.

The record for staff #1 (start date 4/17/2023) did not have a child abuse/neglect registry search for a state the individual has lived in within the past 5 years. 2. The record for staff #2 (start date 5/30/2023) did not have a criminal history search for a state the individual has lived in within the past 5 years. 3. The record for staff #3 (start date 4/24/2023) did not have a criminal history search and child abuse/neglect registry search for 2 states the individual has lived in within the past 5 years.

Evidence: The emergency preparedness plan did not address the following: establishment of center emergency officer and back-up officer to include 24-hour contact telephone number for each; accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety; procedures to reunite children with a parent or authorized person designated by the parent; and continuity of operations to ensure that essential functions are maintained during an emergency.
