Video footage from January 13, 2026, shows Child #1 (age: 16 months) playing on the indoor play equipment. Child #1 attempts to walk down the equipment steps, loses balance, and falls down the equipment steps, hitting their face on the ground and leaving an abrasion on the left cheekbone, underneath the eye . Video footage shows Staff A and Staff B on-and-off their cell phones multiple times, with neither staff supervising the children on the play equipment.
Staff member #1 did not have first aid certification.

Areas and equipment inside and outside the center shall be maintained in a safe and operable condition. On 7/22/2024, the emergency exit door located in the School Age room had a large crack in the glass spanning the entire length of the 6.5 foot glass door. In addition, the door is not able to be completely closed which caused a gap around three sides of the door. The door has been broken for at least the last three weeks at the time of inspection.

Evidence: In the Twos room on 3/26/2024 at approximately 1:29pm, Staff #1 left a red purse containing prescription medication on the children's blue table while the staff member went into the bathroom for approximately two minutes. There were children napping in the room while the red purse was left unattended.
Evidence: On 2/7/2024 at approximately 9:34am Staff #1 quickly approached Child #1 after the child threw a book down, bent down in the child's face with an angry expression, and jabbed Child #1 in the left side rib area.
