Based on staff interview, behavioral guidance shall be constructive in nature and age and stage appropriate and shall be intended to redirect children to appropriate behavior and resolve conflicts. Evidence: 1. Staff #1 confirmed in a written statement and interview on December 4, 2023, that child #1 (3 years old) was knocking over stacked chairs, throwing a chair and toys across the room, and hitting the staff. Staff #1 stated she grabbed child #1 around the waist and sat child #1 down with cross legs. Child #1 was squirming around and refused to calm down and began to struggle. Staff #1 released child #1. 2. Staff #1 confirmed on written statement and interview, child #1 he was throwing shoes, kicking equipment and knocking over office chairs. Child #1 reportedly ran away from the staff . Staff #1 put child #1 in a multi child stroller and strapped into seat until a parent arrived.

Based on record review and staff interview, the vendor did not ensure documentation of orientation has been completed, including the date completed, the total hours of session, and the names of the trainer and of any sponsoring organization. Evidence: On November 1, 2023, the vendor was unable to provide orientation training documentation for Staff #1 that contain specific topics. Confirmed with Staff #2 the documentation with all required information was unable to be located.

During the inspection the center was unable to determine if a out of state centeral registry was submitted to the state he lived in the past 5 years.
