Summary of QRIS Monitoring Visit and Complaint Investigation The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and QRIS Monitoring as well as to investigate complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Items labeled "Keep out of reach of children" were inaccessible but not locked when not in use. Agency is not part of a nationally recognized agency for accredidation. One new staff member has started but all contents of file have not been fully completed. No matierals reflecting language were present in the classroom. Books and other print rich materials were not integrated throughout the classroom. The calendar/weather chart was dated 03/09/26 and did not appear to have been used. Violation cited in regard to complaint investigation for this indicator. Incident occurred on 04/09/26. Violation cited for incident occurring on 04/09/26 for improper incident reporting. It was found that agency did not comply with Duty to Report requirements from an incident that occurred on April 9, 2026. It was found that the agency did not report an incident to DHS and/or DCS on the same day incident occurred. It was found that the agency did not follow proper incident reporting to the parent/guardian on the same day incident occured.

An educator heard a slapping noise, a 3-year-old child cried out, and another educator witnessed the child getting hit again, and the child being pushed to the ground, and was not reported to the parent on the same day of occurrence.
