See Below for Detailed Information Observation: An 11 month old child was observed with a pacifier attached by a strap to her clothing which is prohibited. 1240-04-01-.13 Food and Food Service Corrective Action: The strap was immediately removed from the infant's clothing. The director informed all staff onsite that pacifiers should not be attached to clothing.

See Below for Detailed Information Observation: On 08/16/18 a sleeping infant was not touched every fifteen minutes as required by the rule. 1240-04-01-.11 SUPERVISION Corrective Action: The Director intervened immediately on 08/16/18 and instructed the educator to touch the infant every fifteen minutes. Additionally, the Director has retrained all staff members on Safe Sleep procedures.
