Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation and serving of safety plans for three staff members. This is still an on going investigation and additional violations could still be cited. Technical Assistance Provided: 1. Guided staff through safety plan 2. Spoke to staff members about behavior management Violations were received In the past it was reported they were out of ratio in the pre-k classroom.

See Below for Detailed Information Observation: A child got out of the agency playground gate before the staff could stop him. 1240-04-01-.11 SUPERVISION Corrective Action: The staff should have been in closer proximately to the child in order to prevent him from getting out of the playground gate.

See Below for Detailed Information Observation: The educator inappropriately disciplined children in the two (2) to four (4) year old classroom while on the playground. The educator aggressively grabbed a two (2) year old child pulling them by the arm. Another child was pushed by both shoulders back into line. She also knelt nose to nose with another child yelling at the child using inappropriate language of “why can’t you freaken’ listen”. 1240-04-03-.09 PROGRAM. Plan of Correction: A plan of corrective action was completed on site. The educator was placed on a safety plan. The agency is receiving training from CCR&R.

See Below for Detailed Information Observation: On March 16, 2018 PE Garner observed a six (6) month old baby in a bouncy seat drinking from a propped bottle. The violation was corrected on site. 1240-04-01-.11 FOOD. Corrective Action: A plan of corrective action was left with the owner/director. Discussed that no bottles would be propped from hence forward. Observation: On March 16, 2018 PE Garner confirmed by reviewing time sheets and time cards for the past two weeks that on March 8, 2018, a staff member was alone in the three (3) to five (5) year old classroom with a ratio of 1:14 from 8:10 am until 8:27 am. The ratio in this classroom should have been 1:13. 1240-04-01-.06 SUPERVISION Corrective Action: A plan of corrective action was left with the owner/director. It was discussed with the director that she will ensure that ratios are met at all times. Observation: On March 16, 2018, PE Garner confirmed with classroom teacher and the director that the two (2) to four (4) year old classroom has children ages two (2) to five (5) years old. It was noted a child aged up to five (5) on February 13, 2018 and had not been moved up. 1240-04-01-.06 SUPERVISION. Corrective Action: A plan of corrective action was left with the owner/director The owner/director noted that she will appropriately group the children. Observation: It was confirmed by staff interviews and reviewing time sheets and time cards for the past two weeks that on March 5, 2018, two infants were combined with the other infant classroom prior to the last hour of the day which is 5:00 pm. 1240-04-01-.06 SUPERVISION. Corrective Action: A plan of corrective action was left with the provider. A floater is being placed in the classroom so that the children remain in their assigned classroom. The violation was corrected on site

See Below for Detailed Information Observation: A medication was received by the agency and had not parent authorization form. The medication was administered, and staff did not document when the medication was given and by whom. 1240-04-03-.10 HEALTH AND SAFETY. Corrective Action: A plan of corrective action was completed with Ms. Teresa who noted she would have the parent complete a parent authorization form. Staff were advised to note when medication was given per licensing rules. Observation: Maximum group sizes exceeded in the infant room (5 to 13 months) at 3:9 and in the toddler room (14 to 24 months) at 3:13. 1240-04-03-.06 SUPERVISION. Corrective Action: The violation was corrected on site. One child in the toddler room was picked up by the parent. Ms. Black took her grandchild (infant room) home. Ms. Teresa plans to change her infant room from 6 weeks to 15 months to a 6 weeks to 30 month old classroom.

Corrected immediately. 1240-04-03-.06 SUPERVISION. Violation corrected on site on 7 24 17 with the assessor. Marked corrected on 7/26/17 during the CI visit.
