See Below for Detailed Information Observation: On 11/5/19, the 3-5 year old class walked to a lot to play that was not on the premises of the center. Written permission from the parents was not obtained as required for field trips. 1240-04-01-.05 OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Corrective Action: PE gave Technical Assistance that any time the children are off the grounds of the center, parents would have to give permission for a field trip. The owner completed a POCA and stated the educators would be trained/retrained that children must play only on the playground . This will be completed today. Observation: The 3-5 year olds were taken to a lot which is off the premises of the center to play. This grassy area is past another business that is beside the center. The ratio was 1:13 and was not doubled as required for a field trip, which this is considered, since the children were no longer on the center property. 1240-04-01-.11 SUPERVISION. Corrective Action: PE gave technical assistance to the owner and Director prior to the POCA being completed. It was explained how the actions of 11/5/19 was a field trip that required the ratio to be doubled. Making good decisions as to what is safe was also discussed. The POCA was completed and the owner stated outdoor play would only be on the playground. She will train/retrain her educators to ensure they understand.
