Summary of Safety plan serve, incident and complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and serve safety plan, incident and complaint. LC was informed of an incident where a educator left the infant classroom leaving the room in a ratio of 1:8 that should have been 2:8. LC interviewed staff and Director and it was determined that this occurred.

Summary of weekly probation Visit, complaint visit with SIU DCS. The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and weekly probation visit and complaint with SIU DCS. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. LC observed educator with her back turned and bending over doing something for a few minutes while a table full of children ate snack. An educator left a child outside last month and it was not reported to DHS or DCS. LC found out when a complaint was made about the center. Parents were not notified. LC observed a classroom out of ratio. LC was in the classroom and a child crossed the threshold of the door and was attempting to walk into the next classroom. The educator was unaware the child was trying to leave the room until LC informed her. LC was made aware of an incident where an educator was rough with a child in the classroom and jerked them up with a fork in their mouth. DHS was not told, no DCS referral made, and parents were not notified.

Food was served to childen prior to edcuator being seated and her back was turned while plating the food. Also educator got up with children to assist in washing their hands and left one child at the table with food and her back was turned to him. Educator had children sit down to read them a book but left all food on the table and some children did not sit and were running around the table and could have eaten the food.

Allergy plan is incorrect after two QRIS visits. Two children with no medical documentation of an allergy have it marked they have EPI pens but no EPI pens present. LC asked for allergy list to be updated at the last visit and this was not completed.
-Mealtime supervison plan was not age appropriate for two visits in a row. This was discussed at the previous visit. -During observation an infant was layed flat on the floor to hold and drink their bottle. LC informed the educator that the child needed to be held or placed in a high chair to drink thier bottle. - New employee has not taken the appropriate training to be unsupervised with children. She is in the room with another teacher but can't be counted in ratio and therefore the room is not compliant with ratio. There are 9 children and one teacher plus the new supervised educator.
Lesson plans were out of date in several rooms. This is the second QRIS where lesson plans were not current.
