The attendance records were not available for some of the children. An adult:child ratio of 1:9 was observed with children ages 1-3. Name to face roll call was not conducted when children were transitioned to other areas of the building. A group of children were left unattended when the educator left the classroom to open the door. Children enrolled at the agency were transported on behalf of the agency without approval from the Department.
