Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. LC received a complaint from the State Fire Marshall, stating that she observed an educator leave the children in high chairs unattended in the classroom. LC received a complaint from the State Fire Marshall about the restroom being unsanitary. During the follow-up visit, LC observed that the gymnasium restrooms were, in fact, unsanitary. LC received a complaint from the State Fire Marshall that one-year-olds were watching TV during her inspection. LC observed one-year-olds watching an iPad during her follow-up visit.

If any violations were received, details can be observed in the compliance details section of the visit, along with corresponding corrective action plans. LC received an anonymous complaint about a two-year-old child's arm "dangling" after falling. LC observed the child unable to lift their right arm and moving uncomfortably. After departure, the mother stated that she took her child to the ER for medical attention. The agency did not contact emergency medical attention or DHS.
