See Below for Detailed Information Observation: Three (3) children were not signed in on the parent computer log in and five (5) children were not accounted for on the classroom attendance sheets. 1240-04-01-.11 Supervision Corrective Action: The agency signed all children in. Observation: One (1) teacher was outside with sixteen (16) children ages three (3) years through five (5) years of age. 1240-04-01-.22 Specifics for Child Care Centers Corrective Action: Another educator was placed with the children. Observation: The agency prepared to serve lunch to children that had been sitting on the counter for at least over 1 hour. 1240-04-01-.13 Food Corrective Action: The agency prepared a new meal for the children.

See Below for Detailed Information Observation: Children were being grouped together daily after the first hour of the day. As required by licensing, children can be grouped the first and last hour of the day, excluding infants and toddlers. Violation: 1240-04-03-.06 SUPERVISION. Corrective Action: Provider stated she would have children and staff in their assigned classroom beginning at 7:00 am each morning. Observation: Two children under 2 years of age was grouped in a classroom of children ages 2 years- 35 months. As required by licensing, all children shall remain in ratio with their assigned age group. Violation: 1240-04-03-.06 SUPERVISION. Corrective Action: Violation corrected on site. Provider phoned each parent to have their child picked up from the agency.
