Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. The educator heated a bottle in the microwave. The bottle was not shaken or the temperature checked before giving it to the toddler.

See Below for Detailed Information OBSERVATION: During the Assessment on 03/05/20, the educator looked at the child in the crib and did not touch child while sleeping when timer went off within the 15 minutes of being laid down. 1240-04-01-.11 Supervision Corrective Action: The Director/Owner conducted and retrained the staff on SIDS and the importance of touch not just looking. OBSERVATION: At today’s visit a child did not have a blanket during naptime and due to this being the 2nd time for this classroom a violation was written. 1240-04-01-.14 Equipment for Children Corrective Action: Extra blankets and sheets will be put in all classrooms in case a child does not have one when they lay down for nap.

See Below for Detailed Information OBSERVATION: An infant was not touched within fifteen (15) per rules and regulations of a child falling asleep during the assessment while in an infant seat. 1240-04-01-.11 Supervision Corrective Action: Retraining was conducted with the educator. The Director stated when there is one (1) educator in the infant classroom she or the assistant if able will go in and help more in that classroom.

See Below for Detailed Information OBSERVATION: On an unannounced visit to monitor compliance, Child Care Program Evaluator, Jennifer Walker, (“CCPE Walker”) observed two classes out of ratio. In the twelve (12) to fifteen (15) month old classroom, the ratio was 2:13 and should have been 2:11. In the two and one-half (2 1/2) to four (4) year old classroom the ratio was 1:13 and should have been 1:11. 1240-04-03-.06 Supervision Corrective Action: The agency is in the process of hiring several employees so this does not occur again.
