Corrective Action: The agency staff will be attending referred trainings to address violation.

See Below for Detailed Information Observation: While conducting a complaint investigation via phone call, the director noted that when an incident occurred on 9/9/2020 she alerted the families of both involved children verbally, but did not have written incident reports completed to share with them per licensing rules. 1240-04-01-.09 INCIDENT REPORTING Corrective Action: PE Garner reviewed the rule for incident reporting with the agency via technical assistance. The agency will ensure to draft incident reports for any and all incidents in the future review with families and place them on file.
