Summary of QRIS Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and scoring of the QRIS. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Agency has not had a fire drill since 1/28/2026. Agency must provide a safe enviorment and premises for children. Agency must be in adult:child ratio at all times. Agency must note changes to meals and snacks on the menu in advance. Agency must have a written procedure in place to reduce cross-contamination of allergenic foods. All classrooms must ahve gross motor materials. Agency must report all incidents to DHS Agency must have a variety of materials such as: fine motor, blocks, dramatic play, etc. Agency must have updates lesson plans. It must be observed what the lesson theme is and relates to. All staff must use enriching and educational language with children. Agency must have TNELDS posted on lesson plans. Agecy must have classroom rules posted. During a visit on 4/6/2026, LC observed mutliple classrooms failing to perform Name to Face when transitioning. Agency must have updates lesson plans posted, and one must be able to observe what is being taught. All classrooms must have classroom rules posted. Agency is lacking toys for children. All classrooms must have lesson plans posted as well as TNELDS.

Upon arrival, LC observed the center not having a working telephone.

Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Friday 3/6, staff members had a verbal altercation. What began inside the building then led its way into the parking lot. this altercation consisted of cussing and yelling at one another. During a visit on 3/9/2026, LC could hear an educator from another room yelling at a child to lie down during naptime. Another child in another room was crying for an extended period of time without being soothed. On Friday 3/6/2026, an educator stepped out of her classroom during nap time leaving the 2 year old room with a ratio 1:8 during nap time with the children under 30 months old. Friday 3/6, the center had a verbal altercation with staff members cussing and yelling with the police involved and did not report this to DHS.

Upon arrival, LCs observed a room had inappropriate grouping with ages 1 years-5 years of age. Staff were not getting paid in a timely manner. There have been insufficient checks, and there have been no record of hours worked or taxes taken out. Staff are unaware if they are getting paid for sufficient hours worked.
