Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation. Infants are in restraining devices longer than 15 minutes Infant was asleep in a swing A crib sheet was not the appropriate size for the mini crib mattress.

See Below for Detailed Information Observation: An incident report was not completed as required and shared with the custodial parent after inappropriate discipline was used at the child care. The agency told the custodial parent of the incident the day it occurred but did not prepare an incident report as required. Rule: 1240-04-03-.05 OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Method of Correction: The agency is in the process of preparing an incident report for the custodial parent to sign. Observation: A caregiver's file was incomplete and lacked required documentation. The agency thought that the information may have been misplaced and had to make a new file. Rule: 1240-04-03-.05 OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Corrective Action: The agency is in the process of updating and completing the employee file and has most of the information on file. Observation: A caregiver used inappropriate discipline on an 18 month old child leaving a red mark. Rule: 1240-04-03-.09 PROGRAM. Corrective Action: The caregiver is no longer employed at the center. Positive behavior management training will be scheduled with staff. Observation: The agency initially failed to report to DCS that inappropriate discipline was used on an 18 month old child. This was corrected on 4/3/18 when the director reported the incident to DCS once she became aware that the agency was required to report. Rule: 1240-04-03-.10 HEALTH AND SAFETY. Corrective Action: The agency corrected the violation on 4/3/18 when they made a referral to DCS. Observation: Child care licensing observed that several electrical outlets did not have covers in two classrooms. This was corrected on site on the day of the visit. Rule: 1240-04-03-.12 PHYSICAL FACILITIES. Corrective Action: The child care corrected this violation the day it was observed by replacing the missing electrical outlet covers.
