See Below for Detailed Information Observation: The educator was not supervising children as they ate as required by licensing rules. She was on the other side of the room with children as they went to the restroom. 1240-04-01-.11 Supervision Corrective Action: Educator agreed to immediately come over to where the children were eating in order to supervise. Technical assistance provided. Observation: Licensing observed the educator pull a child the arm across the floor to a nap mat. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT. Corrective Action: The Director and Owner planned to meet with staff on 06/06/2019 to address this violation and review the rule. In addition CCR&R will provide additional training for all staff.

Corrective Action: Agency has several people leave employment in a short period of time. They are in the process of hiring new staff. Additional educators were added to each classroom. Technical assistance given.

See Below for Detailed Information Observation: On 5/3/19 during a complaint investigation, the agency was found to be out of ratio in 2 different classrooms. 1240-04-01-.22 SPECIFIC REQUIREMENTS FOR CHILD CARE CENTERS. Corrective Action: The Director has hired additional staff to work at the agency. The Owner agreed to immediately correct the violation. Technical assistance given. Observation: In the infant room, one child had a blanket in the crib and one child had a burp cloth in the crib which is a violation of safe sleep rules. 1240-04-01-.11 SUPERVISION. Corrective Action: The caregiver in the room was told about the violations and she agreed not to put anything in the cribs with sleeping infants. Technical Assistance given. Director and Owner was notified in regard to violation. Observation: One 2/12/19 during a complaint investigation, the agency was found to be out of ratio in 3 different classrooms. 1240-04-01-.22 SPECIFIC REQUIREMENTS FOR CHILD CARE CENTERS. Corrective Action: The Director has hired additional staff to work at the agency. The Owner agreed to immediately correct the violation. Technical assistance given. Observation: The adult/child ratio requirements were not posted in each room as required. 1240-04-01-.05 OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Corrective Action: The Director agreed to immediately post adult/child ratio requirements in all classrooms. Technical assistance given. Observation: On 5/3/19 during a complaint investigation, the agency was found to be out of ratio in 2 different classrooms. 1240-04-01-.22 Supervision. Corrective Action: The Director has hired additional staff to work at the agency. The Owner agreed to immediately correct the violation. Technical assistance given.
