Summary of Safety Plan Monitoring and Civil Penalties Served Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and monitoring for safety plan compliance. This visit was also conducted for the purpose of serving the civil penalties. Technical Assistance Provided: 1. Reminder to send new playground pictures once new playground is completed If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans.

Summary of Serving Safety Plan Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and serving the agency a safety plan based on violations from the 4/12/23 visit. Technical Assistance Provided: 1. Repairing exposed nails and screws on the playground 2. Adding last names of children on the sign-in/sign-out sheets 3. Making sure parents are signing their first and last names when signing children in and out for the day

Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and investigating complaint #PCS1002728. Technical Assistance Provided: 1. Having an individual clipboard in each classroom for quick referenc e of attendance for each class 2. Completing incident reporting the same day as an incident occurs 3. Making sure playground is safe and playground supervision plan is accurate and pertinent The gate leading from the playground to the parking lot is in poor repair. No classroom had every child signed in during the visit. A parent/guardian was not notified of a serious incident. There was a broken gate on the playground, which allowed a child to escape. A child escaped the playground area and was in the parking lot/near the road. A passerby stopped and got the child and took her back to the daycare.

See Below for Detailed Information Observation: During the assessment it was noted that a caregiver did not touch a sleeping infant every 15 minutes as required. 1240-04-03-.06 SUPERVISION Corrective Action: Program Evaluator reviewed rule and offered technical assistance. Director/Owner will ensure that sleeping infants are touched every 15 minutes as required.
