Summary of QRIS Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and scoring of the QRIS. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Two staff was focused on the children in the 1yr group but did not show awareness of the entire group while working with small children. One 1yr old unsupervised male climbed ontop of the classroom table, fell and chipped his front tooth. No documentation of the observation of TNELDs use available. Technical assistance given Director has completed required training. There is no documentation of parent and family activities available. No citation issued family advisory board required evidence was discussed and reports of activities are reported however it was not documented. Documentation will start at next activity. See Below for Detailed Information Observation: Five children with expired TN Certificates of Immunizations were sign in and cared for between dates March 4, 2019 to March 18, 2019. 1240-04-01-.08 RECORD KEEPING. Corrective Action: Management contacted the children parents and advised to pick the children up due to a lack of proper immunization. Management informed mother the child cannot continue to receive child care without a certificate of immunization. Documentation of immunizations were submitted or the child was picked up and signed out of the agency by an authorized representative. Observation: The high chair equipment manufactures instructions for use were not followed; the infant was not strapped in the high chair. 1240-04-01-.14 EQUIPMENT FOR CHILDREN. Corrective Action: All equipment owner manual for equipment was placed in each class and staff was informed of its location. All staff must follow the owner manual instructions as all times. Observation: There was a lapse in supervision on 03-18-19, when a 6 month infant fell out of a high chair. 1240-04-01-.11 SUPERVISION. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT. Corrective Action: Mealtime supervision plan and rules were shared and reviewed with all staff. Staff must not leave children unattended when children are in high chairs. Observation: Sign in and out sheets for March 7, March 8 and March 12 have children not signed out of the agency. 1240-04-01-.11 SUPERVISION. Corrective Action: All errors on logs were noted and discussed with Director and Designee. Designee was assigned by Director to monitor the logs to ensure that each parent sign in and out of the agency with the time, printed name and signature at arrival and departure. Observation: During meal service, assigned educator was performing other classroom duties not related to food service when a 6-month infant fell out of a high chair. 1240-04-01-.11 SUPERVISION. 1240-04-01-.13 FOOD AND FOOD SERVICE. 1240-04-01-.15 PROGRAM, LANGUAGE, AND LITERACY DEVELOPMENT. Corrective Action: Mealtime supervision plan and rules were shared and reviewed with all staff. All staff must not conduct other duties when children are in high chairs. Staff must comply with the written mealtime supervision plan at all times.
