See Below for Detailed Information Observation: A child’s file does not have an incident report to document the mother was contacted and notified the child had a fever of 103 degrees on June 5, 2018. 1240-04-03-.05 (10)(iii) OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Corrective Action: An incident report will be completed by staff and placed in the child file to correct the violation. To ensure the violation does not occur again, the Primary Caregiver will check all children file to ensure all incidents, accidents and injuries are noted in the child file immediately.
