Band aids were missing from first aid kit in the preschool room.
Three diaper changing table pads were observed to have rips and small holes in them. A teacher had her personal items left out and available in children's reach in the classroom. Extension cords were found entangled and outstretched near the closet door. A sound machine was observed to be placed up high on a cabinet with the cord out and in a child's reach where the item could be pulled down and fall from above.
Health records as required by the Rhode Island Department of Health’s Immunization and Communicable Disease in Preschool missing or not updated for employees.
Child files missing: 1. Written authorization from the parent/guardian for emergency medical treatment; 2. A written order from the licensed physician for medication. 3. Written reports of injuries, accidents, or illness occurring while the child is in the program and any treatment administered, 4. Transition plans, and 5. Two child files were missing two non-parent emergency contacts or a a mandatory reporting form signed by the parent/guardian authorizing the program to act in an emergency.
A storage bin located on the playground was observed to be missing the self-holding hinges, The top of the lid, where the plastic locking mechanism loop sits against the bottom to lock the bin, had a metal bar that was broken off with a row of sharp screws sticking out.
Bedding for cots was not removed in between uses and safely stored in individual plastic bags.
Consent to post allergy information was missing.
A teacher was observed during outdoor play not to be providing sight and sound supervision.
A toddler room was observed to have the cabinet to under the sink left unlocked and access to toxic materials was in children's reach.
Written order from a doctor was missing for medications. Permission from the parent/guardian was missing for medications.
Four staff members are missing the required health and safety training.
Blankets and bedding were observed to be piled on top of cots in Toddler and PS/PK classrooms. Provider shared that she takes all the bedding home over the weekend for washing and brings it back to the center on Mondays. She was not sure where the individual bags were for the blankets that were all piled together.
