At approximately 1:15pm in Room 2 (Sun Room) one staff and three children (2 infants, and 1 young toddler) were observed. During the onset of supervision and ratio, when staff was asked to identify and name the children whom they were responsible for, Staff person #4 identified and named 2 infants thus leaving the 1 young toddler unaccounted for. When asked who was responsible for the one unnamed child Staff person #4 said Staff person #1, who was out of the room on their break, was responsible for that child. Staff person #3, who was in an adjacent room (Room 1, Living Room), also confirmed that Staff person #1 was responsible for the young toddler. Therefore, it was determined that Staff person #1 was observed to not be physically present with their assigned group of children.
The staff file for Staff person #1 contained current staff evaluation dated 6/4/25 which was not completed at least once every 12 months from the previous staff evaluation on file dated 5/20/24.
This allocated unannounced monitoring inspection recorded no violations or advisories.
The staff file for Staff person #1 contained a current Child Abuse clearance dated 1/2/25 and a current National Sex Offender Registry (NSOR) clearance dated 1/3/25 both of which were not renewed within 60 months of the previously dated 5/6/19 Child Abuse clearance and 12/23/19 NSOR clearance expiring.
In the Living Room refrigerator three bottles used by an infant in care were observed to not be labeled with the child's name.

The child file for Child #2, who is a young toddler at the time, was observed to contain health reports that were conducted more than 6 months apart; those health report dates are dated 7/9/22 and 11/11/21.
The child file for Child #1 contained a current financial agreement and emergency contact form that were not reviewed and updated by the parent every 6 months; the two most recent reviews were dated 9/26/23 and 2/4/23. The child file for Child #2 contained a current financial agreement and emergency contact form that were not reviewed and updated by the parent every 6 months; the two most recent reviews were dated 9/19/23 and 1/31/23. Upon review of the facility's 30-day fire testing documentation, the following tests were observed to have been conducted more than 30 days apart: 8/18/23 and 9/20/23.
The file for Staff person #2 contained emergency plan trainings that were not conducted on an annual basis; the last two emergency plan trainings were dated 12/20/21 and 1/2/23.
Room 5 was observed to lack a daily activity schedule.
The child file for Child #1 and Child #2 contained an emergency contact information form that lacked an address for one or more of the individuals listed as a designated release person.
The child file for Child #3, who is a preschool age child, was observed to contain an updated health report dated 10/10/22 which was conducted more than 12 months from the previously dated 6/24/21 health report on file.
Staff file #3 lacked requests for West Virginia criminal and child abuse clearances per CPSL prior to hire. A valid WV Health & Human Resources clearance dated 1/4/22 and WV State Police clearance dated 12/29/21 were in the file. Staff person #3 continued to work with children at the facility without valid out of state clearances from 8/9/21-1/4/22.
The toddler room and the nap room lacked emergency contact forms for randomly selected children who were in care.
Child file #1 lacked a copy of an updated health report dated every 12 months as required for a preschool child.
The garbage can located in the preschool bathroom lacked a plastic lining in the garbage can.
At approximately 10:15am, in Room 3 (Dining Room) which is measured childcare space, one young school age child and one staff person were observed and when asked stated the young school age child was attending cyber school through Commonwealth Charter School therefore the space is being used for a purpose other than childcare.
In Room 1 (Living Room) a spray bottle containing a clear liquid, which staff identified as bleach water, was observed to lack a label specifying the contents.
The staff file for Staff person #1 lacked a valid Child Abuse clearance; the Child Abuse clearance on file is dated 8/14/20 which is more than 60 months old. The staff file for Staff person #2 contained a current National Sex Offender Registry (NSOR) check dated 11/14/25 which was not renewed within 60 months of the previously dated 11/4/20 NSOR check on file.
In Room 4 (PreK 3) a 6-outlet grounded wall tap adapter located to the left of the TV was observed to lack 4 protective receptacle covers and accessible to the preschool age children under 5 years of age in care.
An annual emergency drill was observed to be conducted on 8/19/24 which was more than 12 months after the previously documented emergency drill dated 6/19/23.
The child file for Child #1, Child #2, Child #3, and Child #4 lacked a current child service report every 6 months.
Staff person #5 has not completed the following required pre-service training within 90 days of their date of hire (see LIS code sheet): health and safety training and pediatric first aid/CPR. Staff person #5 has obtained pediatric first aid/CPR training on 1/15/24.
Peeling paint was observed in the following areas: Living room to the right of the loft play area on the baseboard, Sun room to the left of the blue chair on the electric baseboard heater, Dining room on the electric baseboard heater below the window and Room 6 underneath the window on the left side of the room.
In Room 5 an extension cord located to the right of the refrigerator, which is accessible to the preschool age children 5 years of age or younger when they are in that area to wash their hands, was observed to be missing two outlet covers.
The child file for Child #3 contained an emergency contact information form that was not reviewed and updated by the parent at least once in a 6-month period; the dates of the last two reviews were 9/26/22 and 2/22/22.
Upon review of the facility's fire drill log, the following fire drills were observed to not be conducted at least once every 60 days: 11/2/22 -- 1/4/23 (63).

Four windows in Room 6 and one window in the children's restroom, both of which are located on the second floor of the facility, were observed to lack a modification to limit the window from opening 6 or fewer inches.

The staff file for Staff person #3, contained a current health assessment dated 10/9/21 which was conducted more than 24 months from the previously dated 9/13/19 health assessment on file.
The facility emergency plan lacked information regarding volunteer emergency preparedness and continuity of operations included in their emergency plan. The facility also lacked a handwritten policy and procedure identifying the prevention of shaken baby syndrome, abusive head trauma, and child maltreatment.
